Welcome to SHS Sports Medicine!

Students must have a current physical, concussion baseline test, and be registered on FinalForms in order to participate in Stevens Athletics. Physicals should be handed in to the school nurse or scanned and uploaded to FinalForms.

Sports Physicals and ImPACT Baseline Testing Info

Sports Physicals and ImPACT baseline testing are done on an annual basis. Until completed along with registration on FinalForms, athletes will not be eligible to participate in CMS or SHS sports. New athletes and incoming freshmen are expected to bring a copy of their most current physical.

**Please Note**

If an athlete does not have a valid physical, they will be considered ineligible to participate until a new one is completed and handed in to their designated coach, the school nurse, or the Athletic Trainer. This is to ensure athlete safety.

If student-athletes cannot get an appointment for a physical until after their current one is expired there is 14 days in which they can participate. In order to participate during this time the athlete needs to prove that they have an appointment scheduled and their doctor needs to sign off that they are allowed to play. 
 

ImPACT tests are completed prior to the start of each season for all sports as scheduled by the Athletic Trainer. Once an athlete has taken their baseline at the start of their first season of the school year, the baseline scores are valid for the rest of the year. They are not repeated at the start of every season. Look out for announcements on our home page for testing times prior to the start of each sports season!
